HUD-VA Supportive Housing

Program Description

  • Homeless Outreach Services- Often the initial point of contact for housing support for veterans experiencing homelessness or those at imminent risk for loss of housing. The outreach social workers also handle the National Call Center for Homeless Veterans hotline, which can be accessed at 877-4AID-VET (877-424-3838). Walk-in availability at H-PACT/Trailer 37 at VA Providence main campus takes place on Monday – Friday from around 8:00am – 4:00pm. 

     

  • Veteran’s Justice Outreach Services The goal of the Veterans Justice Outreach Program is to avoid unnecessary criminalization of mental illness and extended incarceration among veterans. This outreach initiative involves collaboration among VA Medical Center staff, local law enforcement, and treatment services to provide for the needs of veterans, helping them to avoid subsequent arrests.

 

  • Transitional Housing through Grant and Per Diem Program- VA and community partners transitionally house veterans with the goal of an exit to permanent housing. While in GPD, veterans receive ongoing housing and case management support by GPD program staff. GPD programs are congregate settings; veterans share living quarters, bathrooms, and kitchens and sometimes share bedrooms.

     

  • HUD-VASH Program: Collaboration between VA and HUD where eligible Veterans who are homeless/at risk of imminent housing loss and have clinical care needs are connected to a social work case manager to apply for a Housing Choice Voucher (subsidy) to use in renting an apartment of their choice. The goal is to assist in securing permanent housing, and then maintaining it through community-based support. If Veterans retain use of the voucher, they receive phase-based case management services and additional support from a multi-disciplinary care team, as needed.

Eligibility

  • Housing assistance is open to veterans who are eligible and enrolled in VA healthcare. Veterans not otherwise eligible for VA healthcare may have access to housing support only; our team verifies eligibility when we first encounter a veteran.
