Program Description

The Eligibility and Enrollment Office can help veterans apply for health care benefits. Once enrolled, a veteran will be assigned a primary care physician and a primary care social worker.

Eligibility

• Discharge under other than dishonorable conditions.
• 24 continuous months of service for active duty members serving after 1981 -OR-
• Full period ordered to federal active duty for Reserves and National Guard.
• Not all veterans who fulfill the above basic eligibility requirements are eligible for VA healthcare enrollment. Veterans are enrolled in VA health care according to priority groups, based on disability, service and income.
• Because the eligibility and enrollment requirements are complex, the VA encourages all Veterans to apply so that the VA may determine their enrollment eligibility.

How To Apply

• Veterans can apply for health benefits through form 1010-EZ. The form can be submitted online, by phone, by mail, or in person at the Providence VA Medical Center Eligibility Office. Individual programs or clinics may also assist in the enrollment process.

Documentation

• A copy of both sides of your current insurance card, including Medicare or Medicaid.
• A copy of your DD214.
• Information about household finances.
